Online 'Pre-Registration' With The Practice
To assist people with access to health care during Covid, we are accepting online registrations. Please complete all 3 steps.
Step1 Click here to complete Pre-registration Form (If you do not know your NHS or CHI number just say "NK")
Step 2.Photograph or scan your I.D (A passport (page with photo) or I.D card or birth certificate AND 1 proof of address such as a utility bill showing your name and address) and email to email@example.com
Step 3. Complete the Online Medical Questionnaire For New Patients
If you do not complete all 3 steps your registration will not be processed. Any partial information you have sent will be destroyed.
You should be emailed once your registration is complete, however you can check the status of your registration by calling us on 0141 435 1500. Please allow at least 24 hours between submitting your application and calling to check. If you do not complete all 3 steps your registration will not be processed.
Please note we only request this identification and proof of address in order to trace your previous medical records quickly and to ensure you are currently living within our catchment area. This is in the interests of patient care and safety
