Why some patients are struggling to get a GP appointment

When you are unwell or need support managing a condition, we know that you want an appointment with a GP or member of the practice team as quickly as possible and with a minimum of fuss. But, too often, that is something that practices struggle to provide.

Step 2: Upload scans of photo identity and proof of address

As part of the New Patient Registration and registering for Online Services please upload:

  1. Proof of Photo Identification (passport, driving license or birth certificate)
  2. Proof of Address (Tenancy/mortgage agreement or council tax bill no more than 3 months old) Please note we only require a photo of the page displaying your name and address, not the whole document

You can upload copies of your identity with the form below. If you prefer to submit your documents in person please speak to the reception team.

Please allow 10 days for your registration to be completed. You will be notified when this has been completed. Please ensure that you have enough medication from your old surgery, so that you do not run out whilst waiting to register with us.
